If you participate in the study, you will be randomised to one of the tree treatment groups: IUI with hormones, IVF (normal IVF) or IVF-MNC (minimal stimulation, in which one and sometimes two follicles will grow). We are looking for about 600 couples (this means 200 couples per group), that's why this study is conducted in more IVF-centres. The randomisation (the selection) will be done by a computer; this means we cannot choose one of the three groups by ourselves. If you participate in the study, you can start the next menstruation cycle. You correctly understood that this study ends after you get pregnant or after a period of 10 months. If you didn't get pregnant you will continue with the original plan.
If you randomise for
1. IUI with hormones: you can start directly with this treatment. You can call our centre (between 10.30 and 12.30) as soon as the first day of your period starts. If it is after 12.30, you can call us the next morning. Just like the regular treatment you will have a laparoscopic surgery after 3 IUI cyclus, if you don't get pregnant. The difference with normal treatment (three times IUI in a natural cycle) is that you directly start with hormonal stimulation. If you don't get pregnant after 6 cycles, you will continue with IVF (not as part of this study).
2. IVF-MNC: IVF in a manipulated natural cycle, which means that only one and sometimes two follicles will grown. You'll use just a small amount of hormones. From a follicle of 14mm you will come here every day till it's 17mm (the mean growth is 2mm a day) and we take some blood samples. Right before the punction we will make un ultrasound to see if the follicle is still there, and than we continue with the punction. If the follicle is already gone, or if we can't find an egg during the punction, we can inseminate the semen (like an IUI), so you still have a change of getting pregnant. If you don't get pregnant after 6 cycles (because we use a small amount of hormones, you can start this treatment each month), you will continue with IUI (not as part of this study).
3. IVF (normal): this is like the normal IVF-treatment. You can do a maximum of three cycles, but this study will end after a period of ten months. If you have a lot of cryo embryo's (this means embryo's in de fridge), you won't be able to do complete those 3 cycles. If you don't get pregnant after those ten months, you will continue with the normal plan (which means IUI).
Before you can start with IVF-MNC or we need to take some blood samples of you and your partner.
Do you have a Dutch insurance? The insurance will repay the IUI treatment and the IVF-MNC as an IUI treatment without a maximum. A declaration for the IVF treatment will be send to your insurance company. They repay IVF with a maximum of three cycles. If you don't get pregnant and you need an IVF treatment in the future and you reached the maximum of three cycles, a maximum of three cycles will be paid by the study. If you'll be pregnant (if it is an ongoing pregnancy), the insurance company again repay IVF treatment with a maximum of three cycles.
